Images not showing up on PDF

I have a file upload widget on my form for taking pictures. When I view submissions they are there but when I download the PDF they are not. When the form was first created the images were showing up, now they are not.

Adding images and file uploads should be working once again.  Let us know if you are still unable to add images to your forms or download previously uploaded files and we will be happy to have a another look.
