Is it possible to create an upload form then send the data to Salesforce?

Is it possible to create a file upload form where the recipient can upload a photo and the response/photo be sent to salesforce and notify by email that the form arrived?

As for the notification, this is default in the form. You can create a new notifier by following the steps from this guide: https://www.jotform.com/help/25-Setting-Up-Email-Notifications.
