How do you get the JotForm application to look just like the fillable application?

We currently use JotForm for applicants to complete their online application and submit supporting documents with the application. When I receive the completed form it looks nothing like the form that was completed by the applicant. The information does not let the reviewer know what the relationship of the response is to the requested detail.

How can this be corrected so that reviewers are able to see the requested information and the subsequent response?

I am not exactly sure if I understood your question correctly. Do you want to create PDF report of your submission that will look exactly the same as your form?

To achieve this you can print the form with the users data. Inside of this guide you can find how to do it: https://www.jotform.com/help/335-How-to-Print-the-Form-with-the-Users-Data

PDF report can not look exactly the same as form, but it is possible to customize PDF report inside of PDF designer.

    https://www.jotform.com/help/384-How-to-Customize-PDF-Submissions-Report

In case that you want that text fields from you form are also shown inside of your PDF report you need to check "Show Headers and Texts" option in the form submissions page.

How to show headers or text for contracts/terms/agreements in the PDF format of a submission

In case that I did not understood your question correctly, please explain in more details what you are trying to achieve. Also please provide us ID of the form you are working on.

Hope this will help. Let us know if you need further assistance.

I know how to print the information that has been gathered from the data that the candidate entered into the form. I want to have the completed form (i.e. the one the candidate has filled in from our online application) with the candidate’s information and the corresponding question.

I have attached what we do receive here:

I'm using Adobe Send & Track.
You can view "352094910211185315.pdf" at: https://files.acrobat.com/a/preview/2c6b761f-4c16-4b69-9578-45fd41e69c87

Here is the location of the actual online application: http://form.jotform.us/form/50784755647166

The information that we receive is not easy for the reviewers of the applications to view and understand. We use a committee format for the hiring process and most of the committee members are not trained to glean the information.

Unfortunately, the PDF submission cannot get the same styles as in the form, it can be customized at some point, but only from the submissions view page. So, that is not what you are looking for as far as I understand.
