How can I change the display name of my form?

I copied a previous from to re-use for a different event that subsequently has a different name.  When I have checked the form it still has the original name that is no longer relevant.  I have changed the title of the form but this hasn't worked.

What else can I do?  To start from scratch will be too time consuming.  There must be a way.
