Why did my form disable itself?

I had a form that was done with the exception of adding some images to it. Yesterday instead of saving it flashed that it was disabled. But it still showed the complete form so I saved it again. Today only the submit button and the name of the form was there. It there a limit to the number of fields that you can have on a form? Is that what caused it to disable? If so why not just block from having any more additional fields added? That took me a lot of time to put together. I was going to use it today after adding the images and it wasn't even present other than the name.

I even tried to disable and reenable. But nothing happened it just said it was enabled again even though nothing was there now.

    Hello,

I checked your account and found that it was automatically suspended by the system. The reason for this being is that you have a form which has a title Directory/Social Media login and Passwords. This prompted the system that you are going to use it for phishing, and in which is a violation on our terms of use.

I have reactivated your account for now but please remove the said form title and I suggest to not ask for any passwords on any of your forms, as this will prompt again the system's filter to disable again your account.

If you have questions, please let us know.

    Thanks.
