I have cloned your form into my account for testing, and it looks like the old email notification that was set is out of date (no longer referencing the fields that currently exist on your form).
The autoresponder contains the data from Configurable List widget:
But the email notification is out of date. Please remove the outdated email notification from the form, and simply set up a new one in its place:
The freshly re-created email notification will correctly contain the data from your Configurable List widgets.
Please try it out, and let us know should you experience further issues after that.
Hello, I erased the old email notification and set up a new one but the configurable list is still not showing. Can you please check it to see what is going on? I don't see how it is different than the auto responder set up.

I have also logged into the account that owns the form 60205626600141, and checked its email history logs, and I see that the data from Configurable List widgets is correctly included in the notification emails.
Please make sure that your email software is not stripping out formatting from the emails you are receiving. I hope this helps.
Yes, it is working now because I added a title for the configurable list widget. (Items Needed) Before there was none. I didn't adjust anything on the email notification, so I don't think it had to do with that.
I'm not sure why all of a sudden it needed a title for the widget, because it was working fine for a long time without one.
