Disabled forms. Can you add an image to the holding page?

I am not sure if I understand your query correctly. I have checked your account and see that the account is active in state and in good standing. 

Do you mean to add an image to the message for the manually disabled forms? Please provide us with more information so that we can assist you further. It would be helpful if you can attach a screenshot of the issue to this thread.
