I can't edit expense template

I'm trying to edit the expense form template you have to add some text questions and I can't get the question labels to show up? Is there something wrong with that template or am I missing something?

As I understand you tried to add some extra questions but you couldn't. Here I cloned your form and added a text box and text area fields. Here how I did it:

I think you selected wrong fields to add to your form. I mean you can't create a question field wit a heading or date field.
