we have adult new patient form and have had many complaints that when they click in a box and put in information it is coming back that the field is required....and they put the information in there. Please help...I tried going in and seen where a copy of the boxes were blank and so I added a subtitle to them to get them to show up. A lot of our patients...well...most of them are filling out this form with their smart phones.

The theme applied to your form is causing several fields to be inaccessible. Removing the theme or applying a different theme will allow for the fields to be accessible once again.
