The File Upload field is not showing up

I have included the "upload document" feature, but it is not showing up on the form when I use the link. 

You are correct, I opened your form and I didn't see the File Upload field. I cloned your form and I saw the File Upload field.

I checked your form and I saw a condition related to the File Upload field.

Please delete that since it indicates an error wherein the field being used in the condition is missing or deleted.
