On our contact form (http://www.kicanada.com/contactus.html)The phone number is greyed out for people to add there phone number, because of this problem nobody can send messages to us because it is a mandatory question that has to be answered or it will give this message There are errors on the form. Please fix them before continuing.
Can you repair this?
I also forgot what my login name was .
I have tried all my emails
none of them work
Can you please help me?

The Phone Number field is currently set to "Read-only" which is why your users cannot enter any data there currently. To resolve this, you simply need to turn the Read-only setting to OFF. Please follow the steps below:
1. Click the gear icon on the top right of the Phone Number field
2. Select the Advanced Tab, scroll until you see the Read-only setting and set it to OFF
Save the changes, and you're all set. Your users should now be able to input their number in the Phone Number field.
With regards to your login, it appears that the form you linked and several other forms you created were made using a Guest account (guest_43036803668054). Guest accounts are created when a user creates a form without logging in. You do not have a registered account yet, which may be why you are unable to login. To resolve this, please sign up for a Free account using this link: www.jotform.com/signup.
Once you have signed up for a free account, please contact us again so we can move the forms from the guest account into the registered account.
