Fields in my submissions is empty

I have a troble fields in my submissions is empety! Its not visible!

Your form is working fine. It is capturing the data properly.

Here's my test:

The fields you've mentioned are not required so it is possible for the users to skip them. This is most probably the reason why you are getting empty submissions on those fields.

Making the fields required in the form builder should prevent this from happening again.
