Auto Populate List of Team Member Names

Hi - I am creating an entry form for a dance competition, where each dance studio enters a number of routines. Each routine is entered separately, but I am looking for a way for the team director to essentially create a list of their dancers names & birthdates, and for that list to then appear/populate in each routine entry form they fill out, so they can select which of their dancers are in each routine without retyping - I'd also like for those selections to be used to calculate an average age for each routine (which is how they compete against each other), AND for the NUMBER of dancers they select for each routine to calculate their total entry fee for that routine.
