I have create the Form but is still showing Form Disabled

Your account was auto-suspended by our phishing detector, which is why your form was disabled. However, I checked your form and they don't seem to violate any of jotform's terms. With that in mind, I have now removed the suspension on your account.

You should be able to use your form now without any issue.
