I tried fix the file by re-uploading it in our end, however, it does not work. It seems like that that specific file did not reached our servers during the upload process. This is possible when during the submission, the internet connection of the user at that time had issues, like it is being disconnected intermittently. That can result to a corrupted file ending on our servers. Unfortunately, this cannot be fixed, because the file didn't reached our servers completely. I advise contacting the user directly to get the file.
You may also try re-embedding your form on your website using the iFrame embed code. Here's how to get that code: https://www.jotform.com/help/148-Getting-the-form-iFrame-code. You will need to remove the old embed code before adding this one. We advise using this method to avoid possible script conflicts between your website's resources and of JotForm's.
Let us know if that helps.
