How to a vehicle damage and inspection form

I am attempting to create a vehicle inspection form and post run form. I have a little computer experience and would greatly appreciate a starting point for this project. We have 15 vehicles in our fleet and need to be able to choose the car, the area damaged misssing or broken pre run and then a way to report any damage caused post run. If there would be a way to include a picture in the damage report that would also be helpful. Any suggestions would be helpful thank you.
