Trying to create this form so it can be filled out and submitted online. Is this possible.
Needs to look very similar to the design in the link with all white boxes being able to be filled out and submitted.
I have the form created in a web builder page, plus as a doc plus a pdf that cannot be filled out.

If I understood you correctly you want to create online JotForm form that will look similar to the form on screenshot you provided.
It should be possible to create similar form, but it will require a lot of positioning of the fields, because there is lot of fields that are positioned in the same row.
This guides can help you to position fields inside of your form.
For the bottom part of your form you can try to use matrix field or spreadsheet widget.
Once you create online form you can convert it to Fillable PDF form.
However please note that widgets will not be shown on fillable PDF form.
