Can I add my form to my Facebook page?

Good morning, How can I add a jot form link on a group page on fb underneath my travel shack name?

Thank you for the response, however as I mentioned I have a jot form link on my bus. Page. I’m trying to add it to my favorites under neath my name travel shack.
Here is what pops up not allowing me to add it. Examples of each. I am only using 1 jot form which is on my page not my favorites. I hope this helps. Should I delete the one on my page and start fresh, if so how do I delete it.

When it comes to adding a JotForm form to your "Favorites" in Facebook, the form can be added as a tab on your Facebook Page, and not to your Facebook Profile or to other parts of the Facebook interface.
