I need my forms re-enabled

I am using this website to create a form for my class, our teacher had some requirements as well as a password field. I added it and i believe it is what caused the problem so i changed it to key but my forms are still disabled. My teacher is insistent that i use this website instead of just coding everything myself.

I have added a screenshot of the upload directions she has given us, so that you may view them.

Please note that our forms cannot be used to collect passwords, or any type of login credentials. More can be read in our terms of use.

Any forms that are asking for login credentials are suspended by our system.

I have manually reviewed your account, Tiffany, and as your form is no longer asking for passwords and was not collecting passwords, I have reactivated your JotForm account.
