Payment Form: How to add a text box to each product?

How can I add a text box to each product vs having a text box at the bottom of the form?

I need the customer to be able to give special instructions for each product, i.e. name or initial to be put on that product.  

So far it is not possible to add a text box to each product to capture a description for each one, you can only add a text box to capture quantities only.

If you would like to get customized options for your products you will need to change the current payment type you're using in your form, currently you're using your payment integration to sell products, you will need to switch to the "User defined amount" payment type. 

Then you will need to set up your products by adding the fields, as other questions, this way you are not limited for the payment integration settings and you're free to add the options that you need per product.
