Conditional Formatting Not Working When Pasted to External Site

I have a few If/Then conditional statements set up that work fine on the JotForm site (https://form.jotform.com/63085518460962). 

Pasting the code using the Dreamweaver option into my page (http://www.fibersource.com/feb/Order2017pp.htm) disables them for some reason.

The conditions should change the options available if the user chooses to pay with a Visa / MC/ AmEx or the Wire Transfer option.

Any insight is appreciated!

Try replacing your current embed code with your forms iFrame code:

https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

This will ensure there is nothing currently in your page conflicting with the form.
