continually get incomplete values

I have redone my form about 10 times, and I keep getting an error when I try to submit a form

I tested the form embedded to the page your referenced and was not able to reproduce the incomplete values error.  If your form is still not working on your end at this time, try replacing your current embed code with your forms iFrame code:
