why are my forms disabled?

Hello I added my forms onto my website and they were working last night now this morning they say they are disabled. The code wil not apear on my website can you please let me know whats going on?

I have checked your account, and I am able to see your account has been suspended because of this form: http://www.jotform.us/form/60045573430145. As this form ask the Username and Password:

We do not allow password fields or sign up forms that contains passwords and usernames. It violates our Terms of Use as this kinds of forms had been extensively abused by some of our users to conduct phishing or other illegal activities.

I have deleted Username and Password fields from your form, and activated your account again. So now you will be able to see your form.
