As for the other topic you mentioned about exporting phone numbers to an iCloud address book, I'm afraid that's something that can't be done.
AFAIK, you need the contacts to be in vCard (.vcf) format before you can import them to your iCloud Address Book or Contacts. I found a suggestion by another iOS user from Apple's community forum page here https://origin-discussions-us.apple.com/thread/6175715
This was how he suggested it:
1. You need a compatible vCard format to import them to iCloud
2. If you can create a CSV file (which you can do in JotForm), you can create a Gmail account and import it to Gmail (see https://support.google.com/mail/answer/164640?hl=en)
3. Then export the contacts from Gmail as a vCard (see https://support.google.com/mail/answer/1069522?hl=en)
4. Lastly, import it to iCloud (see https://support.apple.com/kb/PH3605?locale=en_US)
