My form sometimes shows "there are errors on the form. please fix them before continuing"

I make a form with required fills.

But i got reports from some that "there are errors on the form. please fix them before continuing" shown when they try to submit. They said that all required fills are already filled. They only had problem during the final submit.

I am getting a low number of participants and I cannot find the error in the form.

I am worried that other participants are also experiencing this.

Please help to find the error in the form. Thank You
