This is my first time using JotForm as well as creating a fillable PDF. I know the data elements that should be included on my form (i.e. event planning and logistics); however, I am not sure if I should initially create a form in Word then convert to a PDF.
Do you suggest that I create a form in MS Word first?

If I understood your concern correctly, you are wondering if you can create a Fillable Form in JotForm or not.
You can create fillable pdf form in JotForm. Please follow the steps below:
1- Open Fillable-pdf-form-creator. Click "Create a Fillable PDF Form" button.
2- As you see in the image below you can choose one of these options according to your needs. Please check other options in this image. For my case, I choose first option. If you have no existing forms, you can start to create a new form in fillable PDF version like I did in my example.
3- After you create your form, click the Download PDF as you see in the following image.
In other words, you don't need to create a form in MS Word, convert it to PDF and finally convert it to a fillable PDF Form. This process is so long and may be unnerving for you.
My recommendation is to use fillable-pdf-form-creator of JotForm as I mentioned above.
