    how can I set up a notification email for each of the individuals listed in the "Requested By:" field?

    From outlook view. the "From" column should be the name and email of the "Requested By:" field on my form: https://www.jotform.com/?formID=63134225668154

    If I understand correctly, you want the Sender Name and the Reply-to Email Address on the notification to be based on the items selected in the "Requested By" field? It looks like your Requested By field is a widget. Unfortunately we cannot use the Dynamic Dropdown widget for your requirement.

    To achieve your requirement, I would suggest using a regular Dropdown field instead.

    Once you have them as Dropdown fields, you can then change the Notification settings based on these fields. For more information, you can also check out our guide here: Setting Up Email Notifications

    Thank you for your help.

    It works, but as you can see in the image of my outlook inbox, the sender name is correct but the image also says the sender email is Olga<bill.talero@gmail.com>

    I would like it to show this:

    From: {requested by} follwed by {email}

    i.e.

    from Olga <wpb@scenhound.com> or

    from Olga <jupiter@scenthound.com>

    The sender should be same as {requested by:} but the reply to should always be the email for the location where work was performed.

    Hello Bill,

    If I understand your question correctly, you need to display "Requested by" along with the email address "{email}" as "Sender Name". Is that correct?

    One way to achieve this is by adding a hidden field in form and populate is with the value of "Requested by" & "{email}" field. You can then set this field as the "Sender Name" of your form's email alert. Please check the screenshot below:

    It is the space (empty space) if you, for example tap space button in keyboard, while in Form calculations widget wizard, it will insert that square.
