We used your service and sent email couple of years ago.
I want to reactivate the service... where do I get my old customer email ids?

I cannot understand what you want to refer to "old customer e-mail ids"?
However, I checked your account and your account's status was "DORMANT".
I activated your account again. You can log in your account and if you didn't delete your forms, you can view your all forms which you created.
If your problem isn't solved, please reply this topic.
