Account is Suspended

My account is VBSBusiness. I am unable to access my forms or even log in. Please let me know why my account is suspended and what i can do to resolve the problem. 

 

One of your form is used for phishing. It is asking for login information and credit card information. If you want we can delete the phishing form and restore your account.

This is a form that we created for our new clients to send us login information
to their platforms. Is there another way to do this? We have to be able to
collect this information from them.
Additionally, we note for them to call us with Credit card information. We are
not asking that they submit it with the form.

You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
