How do I change the title of the form which shows up on the server tab?

Please see the photo attached.  How do I change the title in the servers tabs (circled in red)?

To change the form title you will need to click on the preferences icon on the top toolbar while in the Form Builder: 

Once there go to the advanced tab and click on the show more button: 

Scroll down a bit and you should find there the option where you can set up the proper title you want to show in the browser's tab:
