How to position heading and logo in the middle of the form?

Oh and while I am admitting to my 'lack of capabilities' I can't seem to
get the title of the eval in the middle of the page and the logo I added in
the middle.

To position your heading, select your text field inside of Form Builder and click on Edit HTML button.

Then click on Source Code option and change padding of your header from 15px 40px to 15px 10px. Then click on OK and on Complete.

To position your logo simply select that field and set Align to Center. 

After that your form, will look like this.
