I have a leave form , and i have 22 staff each one has leave accumulated i want to put it in the form

1. I want to put in the Form if say John Doe ( i have 22 staff)

2. at end of everymonth i will add to John doe say 23 days of leave  

2.CAn i put a pin number and  he after he puts in his pin number he wills  see the number of days he has remaining and will send it to the admin along with the Form
