Multiple Payment Form

I'm trying to create a multiple payment option for a nonprofit to receive one time and recurring donations both using DWOLLA.

This is the Primary Form

I'd like the primary payment form to only show the Header and the choose the donation type radio buttons.

Depending on the user input, then the second corresponding form should be shown. IE One time donation selected then the one time donation form should display.

Also on the recurring form I'd like to be able to expand the width of the text (100.00 shows up as 100. with part of the 0 showing)
