Why my forms are disabled?

I made two jotforms that are linked from this site. However, it says the links are disabled. Can you please help me figure out how to fix this? 

The links to click are "unlock modules" and "resend password"

I  checked account associated with those forms and I found out that that account is auto-suspended and that happens because of our anti-phishing system that detects and disables form with suspicious fields, labels etc.

The affected form is: http://www.jotform.us/form/63185705999170 because of these labels:

I changed those fields to TEST so your account won't be suspended again. Please avoid putting those words to your form and change them to something else.
