Form is not showing up in the checkout process

I created an upload files form for my customers to be able to upload pictures to my website (on shopify).  I added the embed script to my settings-> checkout code-> order procession (additional scripts).  And when I test my site and go to checkout, I do not see an upload files form.

Unfortunately, I can't open your website, it requires a password. I would love to do some testing before answering your inquiry.
