Included the mobile responsive widget in the form. Works great on Android.  Not so great on an IPhone.

I've taken a deeper look into this issue for you using my iPhone 6 on iOS 10 in Mobile Safari and what I found is that the form itself is mobile responsive as it should be displayed on mobile devices.

However, when embedded on your website it's notable that the form is attempting to be mobile responsive but isn't and get's cut off unless you refresh the page which leads me to believe that something in your site is blocking the mobile functionality of the form from working properly.

It appears you've embedded your form within an iframe using the scripted method upon looking at it so I would suggest trying out replacing it with just the iframe method to see if that helps at all and if not then we can try other options.

Alternatively, it's possible to try to manually correct it via our form designer or injected css targeting mobile devices but, this may take some time to make any appropriate corrections so that it displays right if you wanna give that a try.
