How to hide conditionally shown fields if another field's answer is changed?

    In diabetes question(last radiobutton), if you select YES, then one of the options(eg Type 2 on Insulin), then go back and select NO, it still leaves the text fields (eg Insulin dose and Medication).

    How can i hide the 2 textfields (Insulin dose and Medication) completely when i click 'No'

    The "Insulin (dose)" and "Medication" text fields show because of the "Diabetes Type" radio button remains filled even if it's hidden.

    You can update the condition that shows the two fields and add another rule.

    Then, make sure the rules are ALL met before the fields are shown.
