Occasionally I will not receive the PDF form they've submitted. I use a form to track a patient 1 and 4 week post op. I always receive the 1 week follow up PDF submission but sometimes when they use the same form again for the 4 week follow up I do not receive it. I 've checked my spam folder...not there. The user tells me that they've received confirmation that it was sent but I did

Could you please confirm that the form entries are being properly delivered to submissions, and only emails are missing?
Correct. Once in a awhile I the emails will come up missing.
I assume this was the form https://www.jotform.us/form/60213463341141 that have the issue. Please correct if not.
I checked the sent email logs from the submissions of this form and so far they showed that emails were being sent successfully.
Can you share to us a few existing submissions of the form which you had identified that did not send the email.
We will review what could have happened to the missing emails of the submissions.
I also suggest to check in your mailbox Spam email folder if perhaps jotform emails were filtered and redirected there. If you found jotform emails in the Spam email folder, please mark them as Not Spam email as well.

There is also a chance that the email delivery on upload forms might be affected by enabled PDF Attachment option. Our server is supposed to include images to the generated submission PDF and forward it with the email. This might be an issue when the form user is attaching several files.
You might consider disabling the PDF Attachment option and adding the submission PDF download link directly to the email body.
