Problems with Conditional Formatting

    I have three check-boxes (separate fields) for which I am trying to apply conditional formatting. When any of the fields are checked, I would like for it to then show other specified fields and require completion. I have added conditional formatting for this, but it does not seem to be working.

    I have checked your account and I can see you have three different conditions that affect the same fields:

    I can also see you have three different fields for this, could be possible to add only one check box field and the three options? 

    If you still require to have 3 different fields then I would suggest you to have one condition only, in the statements you can add more than one field so you could add the three check boxes that you have in your form:

    So the last condition should look like the one below, where you evaluate the three fields and show the other fields if one of the check boxes is filled:
