People are saying they can't access the form from their computer but they can from their phone. What might cause this? Would it help to add the mobile widget?

I opened your form on my side and could not replicate the same problem. I was able to see the whole form on both Chrome and Safari browsers.
Could you provide us the browser details in which users are having problem with for further inspections?

ANDROID PHONE, WINDOWS TABLET, iPHONE. i DO NOT KNOW WHAT THEY ARE DOING.
