I have just set up a new form and have selected that attach PDF option in the email autoresponder but for some reason no PDF is attached. Could you please advise?Page URL:

I have cloned your form. I have seen your Autoresponder settings and you want to send the emails to the one of the choices on the dropdown field called "Clinicians Email Address". I have changed this field on the cloned version of your form to my personal emails.
I ran a test submission with these changes and I managed to receive the autoresponder email with the PDF attached:
However, since there is an "other" option on that particular field of yours this creates a possibility of the autoresponder not sending at all. But other than that everything seems to be in order. I suggest you to delete your current Autoresponder setup and add a new one with the PDF attachment option enabled.
Please test this and let us know if the issue still persists. We will be waiting for the feedback.
