This is a valid question since usually when a form has been disabled is because of phishing suspicious.
I have checked the form in your account and I can see this is the case, you're asking for Credit Card info in your form and our anti-phishing detected this so your form was disabled and account suspended.
Here's a screenshot about the guilty fields in your form:
This is also stated in our terms, please check them here so you can know what you can and cannot ask in your forms: https://www.jotform.com/help/8-Terms-of-Use
In order to re-activate your form and account I had to remove those fields from your form, please check your account and form since it should be working now.
If you need to ask for payment info you will need to use one of our payment integrations, you will find them here: https://apps.jotform.com/category/payment_processing
