This is a valid question since usually when a form has been disabled is because of phishing suspicious.
I have checked the form in your account and I can see this is the case, you're asking for Credit Card info in your form and our anti-phishing detected this so your form was disabled and account suspended.

In order to re-activate your form and account I had to remove those fields from your form, please check your account and form since it should be working now.
