When i first intergrated my form with Box it would save form info & attachment in a folder. But for some reason it stopped doing so.

In your form https://www.jotform.us/form/62524714931152 have you already tried redoing the integration process to Box.com? If not yet can you try it for now.
Remove the existing integration, save the form and then re-integrate it again to Box.
See if this process resolve the issue already.
I have just checked Box.com integration on my form, but so far it was working just fine.
Please let us know if issue still remains.
