For some reason the dates that are on my form are being filled in on the form. When the completed is received by myself the dates appear as blank, all the rest is received.
Can you help?

The field names on your notification are different from the field names on your form, and as such the dates will not appear in the email.
You can add the appropriate field names for the dates by clicking the Form Fields dropdown and selecting the Arrival and Departure Dates.
Or you can simply setup a new email notification, which I see you've already done.
