The first form I am creating is showing as form disabled when trying to preview

The One Time Payment Authorization form in your trash folder is causing the auto suspension. The form is asking for credit card details which is against our Terms of Use (http://www.jotform.com/terms/). Please delete the form from your trash folder and make sure not to create the same form again.
