Shipping form not adding correctly

We need to have shipping add charges to each item.  As it stands now it only adds one charge no matter how many items we enter.    The items do add up correctly but it only adds one charge?

Example;  When entering 3 items for campfire coffee sets the amount for each set is added correctly.  But the shipping charge only adds one charge.

Any help with this would be greatly appreciated.
