Required Fields not showing up on submissions

Hi

ON our form we require student's name and information.  We have received 2 submissions that do not contain the required fields in the widget.  I have name the widget required.  

Please help

I checked your form and the required fields look to be working correctly. 

Fields in the form are only required if they are shown.  Since your image radio field is not required and the student information field is only shown if that field is filled, if you do not fill the first field, you can bypass the second.  This is what happened with your submissions that are missing data:

If you require the image radio button field to be filled, the form should no longer allow for bypassing entering student data.
