I have checked your account and I can see you're account is currently under the bronze yearly plan, your subscription is being paid through PayPal, please do note that this is an auto-renewal service and your subscription ends on December 31st, 2016 so it means that on this date the subscription will be automatically renewed, this is also stated on our pricing and terms pages, you can check it here:
If you want you can cancel your subscription on the PayPal account, this way the subscription will be cancelled on the renewal date and you will not be charged anymore, here's a guide that will help you with this: https://www.jotform.com/help/336-How-Can-I-Cancel-My-PayPal-Subscription
