Why is Tumblr integration inexplicably broken?

My page, contact.aetoricdesign.com, is a site hosted by Tumblr with Platform integration by JotForm. As of today, it no longer shows up on my Tumblr page. I have since tried several option provided by JotForm trying to fix this, with no success.

Please help. This service actually helped me!
