Duplicate submissions rarely happens when the user is experiencing problems on the initial submission or when they intend to reload the page while processing the submissions. There are also cases that during the submissions, when the browser back button is used, it might create duplicate submissions specially on embedded forms.
However, there are ways to stop this by enabling unique submission in your form. This means that users within the same IP or using the same browser will not be able to submit the form twice. You can also prevent multiple submissions the same name, email or any other field you want to filter.
